| Dear Parents/Guardians, Welcome! If you have registered your child for a week of Creativity or Clay Camp, please be sure to complete an Online Medical/Release Form. This form is due one week before the start of the camp week your child is attending. This form and any additional information specific to each camp week will also be sent via e-mail to all parents/guardians two weeks before the start of the camp week their child will be attending. |
| Medical/Release forms will be kept on file for the duration of the summer camp season. If you have already filled out the form for the summer, you do not need to do so again If your child attended any camps during the winter/spring season, you will need to submit a new Medical/Release form for the new summer season. |
